Enrolling in the Institute is simple. Please print and complete the two documents below and mail them to Chris Brooks, or hand them into one of the members of the Institute's faculty. Please be sure to include your payment along with your forms. Checks should be made payable to Souhegan High School with "SHS Institute 2008" written in the memo.

The documents provided require Adobe Acrobat Reader to open. If you do not already have the program it can be downloaded for free by visiting the Adobe website.
